For a phase of life that every woman goes through, it is astonishing how little we are told about it.

So what exactly is perimenopause?

Perimenopause literally means "around menopause". It is the years-long window during which your ovaries gradually wind down and your hormones — particularly oestrogen and progesterone — begin to fluctuate and decline. The key word there is fluctuate. This is not a smooth, steady drop. Your hormones can swing wildly from one week to the next, which is exactly why the symptoms can feel so unpredictable and confusing.

Menopause itself is a single day — the point at which you have gone twelve months without a period. Everything leading up to that is perimenopause, and it can last anywhere from a couple of years to a decade. Understanding that you are in a transition, not a permanent new state, can be reassuring in itself.

The symptoms nobody warns you about

Most of us grew up thinking menopause meant hot flushes and that was about it. In reality, fluctuating hormones affect almost every system in the body. Here are the ones I hear about most from the women I work with:

  • Mood changes — irritability, anxiety, low mood, or feeling unusually tearful.
  • Brain fog — forgetfulness, losing your words, struggling to concentrate.
  • Hot flushes and night sweats — sudden waves of heat, often worst at night.
  • Weight changes — especially stubborn weight settling around the middle.
  • Irregular periods — closer together, further apart, heavier or lighter.
  • Aches and stiffness — joints and muscles that feel older than you are.
  • Fatigue — a bone-deep tiredness that sleep does not seem to fix.

Seven ways to ease perimenopause symptoms naturally

Here is where I get genuinely excited, because so much of how you feel through this transition is within your influence. These are the strategies I come back to again and again with women in midlife.

1. Build every meal around protein

Protein steadies your blood sugar, protects your muscle as oestrogen declines, and keeps you fuller for longer — which helps with the midsection weight changes. Aim for a good source of protein at every meal, roughly 25–30g, starting with breakfast. This one change alone can smooth out energy and mood dips dramatically.

2. Lift weights

Strength training is the single most powerful thing you can do in perimenopause. It protects the muscle and bone that falling oestrogen puts at risk, lifts your mood, sharpens your brain and helps manage weight. Two or three short sessions a week is plenty to start.

3. Protect your sleep fiercely

Poor sleep makes every other symptom worse. Keep your bedroom cool and dark, wind down without screens for the last half hour, and keep a consistent bedtime. If night sweats wake you, breathable natural-fibre bedding and nightwear genuinely help.

4. Lower your stress load

When oestrogen falls, we become more sensitive to the stress hormone cortisol — which is partly why anxiety and belly-weight can creep up together. Daily stress-lowering habits are not a luxury now; they are medicine. A walk outdoors, slow breathing, gentle yoga, or simply five quiet minutes with a cup of tea all count.

5. Be honest about alcohol and caffeine

Both can trigger hot flushes, disrupt sleep and worsen anxiety during perimenopause. You do not have to give either up entirely, but noticing how they affect you — and cutting back, especially in the evening — often brings quick relief.

6. Stay hydrated and keep cool

Good hydration supports energy, digestion and skin, and helps you manage temperature swings. Keep water within reach through the day, and dress in light layers you can peel off when a flush arrives.

7. Move your body every day

Beyond structured workouts, simply moving more — a daily walk, taking the stairs, a gentle stretch — improves mood, sleep, joint comfort and energy. It does not have to be intense. Consistent, joint-friendly movement is the goal.

When to speak to your doctor

Lifestyle changes are powerful, but they are not the whole story, and you deserve support. If your symptoms are affecting your quality of life, please do book an appointment. The habits you build now — strength, protein, sleep, calm — do not just ease symptoms; they set you up to feel strong, clear and vital for the decades ahead.
